In this stage of life, you and your child begin planning for adulthood and the future.
What kind of job would they enjoy? Where will they live? Our transition SSA will work with you and your child to identify needs and set goals while continuing to assist with community resources and working collaboratively with your child’s school for services and IEP.
We also offer job skill development programs for people in this age group, including Summer Experiences and Opportunities for Ohioans with Disabilities.
If eligible for our services, you will be assigned an SSA who will assist you in planning for your future and finding community resources to meet your needs.
Success Story
Meet Zachary, a hardworking and fun-loving individual who recently wrapped up his high school career at Wauseon High School. During Zachary’s senior year he participated in several community based vocational training sites. This allowed Zachary to learn various job skills and experience different job tasks to identify his likes and dislikes. The training took place in settings such as Goodwill, Chief Supermarket and Fulton Manor. Zachary’s favorite was at Pizza Hut where he was responsible for panning the pizza dough. Zachary’s production at this site was phenomenal and it impressed the management staff. Zachary loved his job tasks and looked forward to “going to work.” After completing the training at Pizza Hut in March, Zachary pursued a permanent position with support from school and HOPE staff and began working part time in April while finishing his academic career. Zachary received on the job supports from Supported Employment until he graduated in May. He now receives on-going supports from COREe. In addition to his part time position at Pizza Hut, Zachary also works five days per week at Triangular Processing Inc. In his spare time, Zachary enjoys watching Disney movies, swimming, spending time with his family/friends and shopping at Goodwill. Congratulations Zachary!
